To secure the University Certification of Education form from the Master of Social Work Program at Chicago State University (IDFPR documents), you must have completed the MSW program at Chicago State University.

1. Using Adobe Sign, complete the (IDFPR) ED form. No further action is required. If approved you will receive CSU's signature and seal on the document you submitted.
2. Visit the  Website for more information or need more detailed instructions.

The form will be available for pick up in the Office of the Registrar, Cook ADM Building Room 128, CSU-Registrar@csu.edu.  You will receive and email from the registrar when the forms are available for pick-up.
